To complain to, or request information from, the Ombudsperson for British Columbia (Canada), use the secure form below. Before you fill out the form, we strongly encourage you to read the information provided on this web site regarding what the Ombudsperson can investigate.
The online secure web form is designed to be a brief summary of your complaint (not more than 3,000 words). For a more detailed submission please use one of our other forms that you can print and mail or fax to us.
The "Submit" form button will transmit your form in a secure and confidential web connection. You will be contacted within ten working days by telephone, fax, or by Canada Post. The Ombudsperson will not reply by e-mail.
