The online complaint form is designed to be a brief summary of your complaint. For a more detailed submission please print off one of our other forms and submit your information by mail or fax to us – or call 1-800-567-3247.

After you submit your complaint, you can expect to be contacted within five working days by telephone, fax, or by Canada Post.  Due to security concerns we do not reply by e-mail.

  • When complete, please save or print a copy of your complaint for your records.
The Ombudsperson Office hours of work are Monday to Friday, 8:30am to 4:30pm. Please indicate if you have a preference for the best time for us to call you?
Give us the name of the organization you are complaining about. (Please be as exact as you can be.)
Give us the names of the people you have dealt with there. Give us any job titles, phone numbers or business addresses that you have for them.
Tell us your complaint. Tell us what you have done to try to solve the problem yourself. (Please give us any file or reference numbers and the dates these things happened.)
If yes, when was the last appeal or review and what was the result?
Why do you believe the organization's actions are unfair?
What do you want to happen? Describe the result or outcome.
If you consider the matter urgent, explain why.
